Need code for conditional question and “other” response option

Hello! I am a speech therapist who needs help with a question on my web site’s intake form.

Can someone please tell me what code I could use to create a conditional question based on a yes / no response within Contact Form 7?

When ‘yes’ is clicked for “Will you need a statement to submit to your insurance for reimbursement of services?” I would like a second question to pop up which asks “To which insurance carrier will you submit?” I need 10 radio button response options reading: Anthem Blue Cross, Blue Shield of California, United Healthcare, Primera of United Healthcare, UMR of United Healthcare, Aetna, Meritain of Aetna, Cigna, Health Net of California, and Other: _______[with a text box for them to write in]

I hope to be able to cut and paste the code into my form … please don’t assume I know coding. Anything I know is self-taught through studying the screen at the midnight hours … !
