Something is preventing the JavaScript from loading – most likely a conflict with your theme or with another plugin. Take these steps to find the conflict:
1. switch to an unmodified default theme such as Twenty Fifteen
2. de-activate all plugins except Awesome Surveys
the JavaScript should load now. From there, you can slowly re-activate plugins one-by-one to find which one is causing the conflict. If none of the plugins caused it, then re-activate your theme.

I just recently downloaded this plugin I have not had any problems with it I found out about this survey plugin on YouTube what I like is it is very straight forward very easy to use.
What does concern me is how much longer can I keep using this plugin or by any chance is there a new one being made and how much will it cost.
Yeah sure there are many Quiz & Survey plugins out there but I am only interested in a basic straight to the point survey plugin like this.
I have a new site just for surveys with very few plugins and so far so good.
It would be great to have some feedback if possible thanks.
