MT Import: No Submit Button on Step 1

Today I upgraded from Mingus to Strayhorn on one server (we’ll call it “mint”), and installed fresh on another that was running MT (we’ll call this one “plaid”). To be thorough, I actually treated each as a fresh install, deleting all Mingus files from mint and doing fresh installs of Strayhorn on both mint and plaid. I simply used the wp-config.php to point to all the existing posts in the existing database on mint. On plaid, I followed the directions to import the MT posts. Here’s the problem and point of this thread: After hitting “let’s go” and proceeding to Step 1, I am presented with a screen asking how to handle the existing authors. Everything on the page is normal, but there is no submit button; I can not proceed to Step 2! I have experimented, placing the import.txt on mint to see if the import-mt.php file renders the same result on that server, but it provides a submit button after the last author. To be clear, I did edit the import-mt.php to point to the import.txt.
If it means anything, on the server in question (plaid), I could not install Mingus because it was one of the many instances I’ve read in the forums where the password would not generate on the final install step. That wasn’t an issue with Strayhorn.
Any ideas why I can’t see that submit button, or what I might be able to do about this? Any workarounds?
