 * Hello, maybe with this [http://codex.wordpress.org/Function_Reference/do_shortcode](http://codex.wordpress.org/Function_Reference/do_shortcode)
   but i have never tested it.
 * The supported ways of adding a form is via a shortcode or a widget, please check
   the video tutorials if you want to see a demonstration.
