 * Hi.
 * I am searching for a plugin in that will help me create a “COVID-19 self reporting
   website”. The functionality I am looking for is as follows :
 * 1. New users must be able to register their profile – I have done this with UM
   
   2. Users must be able to log in daily or whenever they intend to go to their 
   place of work 3. When they log in they should be directed to a self -reporting
   form. This will have 5 or 6 questions on it 4. Once they have completed the form
   they submit 5. Once submitted they need to be sent a confirmation email 6. The
   email must have a QR code on it , which will be made up of their ID number and
   the date they self reported
 * I managed to setup a registration page with all the info I need using UM. I have
   also done the log in, log out etc etc but the difficulty I have is in creating
   a form that has the functionality to required to generate the email with the 
   barcode.
 * Can anyone help me here? Is UM the right plugin for this? Any suggestions would
   be greatly appreciated

 * UM can handle those membership functionalities that you need but for the QR code
   generation and custom email notifications, you will need to customize the UM 
   plugin with its filter and action hooks. Here’s a tutorial on how to create a

 * For the Questions and Answers forms, you will need a third-party plugin such 
   as WP Form or Formidable Form Builder. This plugin will allow you to attach those
   UM Email notifications when the Question/Answer form is submitted successfully.
