Adding on WP Login/Register Page?

Currently the plugin only shows up in the http://www.mysite.com/wp-admin URL. How can i get it to show on the user/customer login/register page? (www.mysite.com/my-account)
I want to add this plugin like the example below
http://olivefountain.com/?page_id=392
How can I achieve this?
