It seems like Conditional Fields plugin is not activated on your website. Could be a problem with caching too.

well, the best way to check might be to disable all your plugins except cf7 and conditional fields. does it work then? if so try to gradually enable plugins again until it fails. the last plugin you enabled will probably cause the issue.

If not, then there might be some caching enabled on your server. If so, you could try and contact your host

Well, something is definitely preventing CF7 Conditional Fields to load.
Could you try and switch themes?

Other plugins and themes can hook into the CF plugin, and prevent the scripts and styles from being loaded. See here: https://conditional-fields-cf7.bdwm.be/docs/faq/can-i-load-js-and-css-only-when-necessary/

You might want to scan all your WP files and search for the code here.

Another cause might be that the plugin is dynamically disabled on page request.

What I would try if all else fails, is the following (this is pretty drastic though):
    – Create a fresh WP installation with a basic theme.
    – Install CF7 and Conditional fields, and verify it’s working
    – Gradually install your theme and other plugins and after enabling each one, verify that the form is still working
    – Transfer your content to the new site.
    – Redirect users to the new site
