This error can happen due to different reasons but in general, when you see that error it means there was an error background that prevented the form from being submitted successfully.
Since maintenance is enabled on your site I couldn’t check the form and find any clue, is it possible to disable the maintenance mode for a while and let us know here so we can take a closer look?
Also, I would suggest checking the site error logs, you may be able to find more info in your site error logs, please take a look at this document to learn more about how to enable debug mode and where to find the logs:

sorry, i answered to the email. Looks like it didnt arrive.
The Website is online right now. So you can take a look at it.
The form is also working. So we recieve the filled out form. The problem is, that the form shows an error to the user, even the error does not exist. The user is confused because of it and it leads to more submittion with the same information.
we installed an smpt plugin.

Both of the forms have the same issue.

We tried to replicate the issue across multiple locations and different browsers but all the tests worked.
I noticed some ReCaptcha issues https://monosnap.com/file/qFJFZfIIwpU8MhNvrYEHIWVT7MjPYz coming from cmsmasters-elementor-addon plugin, could you disable that plugin and see if makes any difference?
When the problem happens to you, can you also check the Ajax response?
For that, click on the right button of the mouse > inspect, switch to the network tab, fill your form and clear all the existing requests:

An update here
One of our agents could replicate some issues in the Appointment form, it seems random and the return is “Failed to load resource: the server responded with a status of 500 ()”
This is a fatal error return, in that case, please enable the debug mode following this guide https://wpmudev.com/blog/debugging-wordpress-how-to-use-wp_debug/
Try to replicate the problem again, it won’t fix anything but once you replicate it, go to WordPress folder > wp-content, and locate the debug.log, open and find the line where it says “Fatal error…”
