# Basic Form Upload Help

 * I am trying to create a form that has three features:
    a customized title upload
   file option sumbit button
 * Anytime I try to modify the code, I get multiple “upolad file” buttons, but I
   do not see in the code.
 * Any help on the appropriate code would be appreciated!!
