 * I’m looking to optimize the layout of the form as it is unsightly in parts. –
   Please assist.

    – Uniform width
   of calendar and entry fields – Fix offset of weekdays in calendar, further to
   the right – Custom accent colour for weekday and hour selection – Custom look
   of button with accent colour – Columns for field entries and button / captcha
