So it breaks just one Gravity Form, but not the others? That’s odd. Are you using any different sort of field type or anything that might make that one form unique?

There should be nothing that conflicts with GF, and I’ve been very careful to name functions with a unique prefix, but anything is possible.

After recreating the form, turns out the Gravity Form’s Multi-Select field type is the cause. I changed it to a Check Box list and all is well. What’s funny is that I have a second Multi-Select list on the same form, but that one doesn’t break it. No idea why.

Weird. I’ll add it to my “watch for this” list and we’ll see if we see anything else similar.

    Hi David,

I see you added this to your watch list 7 months ago. I added seamless donations and it completely blocked all of the forms on our page. We do require the multi-select option on our forms, so I unfortunately had to disable seamless donations. I was hoping you could look into this, and see if you can resolve the conflict it has with gravity forms multi-select. If not, I’ll have to submit for a refund for the premium add-ons we subscribed to and find a different plugin for our donation needs. I love the simplicity and appearance of seamless donations, so I am hopeful that you can find out why it blocks gravity forms.

Go ahead and shoot me a note (hopefully with information about your orders) at http://zatzlabs.com/contact-us/. I’ll issue you a refund.
