This is browser caching. Are you using another optimization plugin? Can you check your htaccess whether another browser caching rule exists or not plesae?
Sorry, I just noticed the JS files aren’t combined by WPFC.
I suspect all is good now.

When I clear the cache does WPFC create new filenames, so it forces browsers to download the new files?

yes, but if you add a browser cache for html file, you need to do CTRL+F5

it seems to me this problem is not related to browser caching, rather server side FCGI cache on NGINX.
When FCGI cache is ON, when you delete the WPFC cache visitors still see the old version for some minutes. I switched off browser caching and the problem has not solved.
I have noticed this since last update of WPFC plugin on my site & seems to affect other sites as well. When you delete minified css as well, the site loads w/o css for some minutes.
Swtching of FCGI cache on cPanel -> everything works fine again.
Bjorn, can you check & confirm you have the same issue?

ow, I did not know it. you should not use a server-side caching system.
