 * I am developing a plugin and I have trouble activating it. I don’t know what
   is causing the error and I just can’t seem to find a way to actually display
   the error. The only error message I get is: Plugin could not be activated because
   it triggered a fatal error. Below that is a space (an iframe from what I could
   see in the source code) and when I try and display that iFrame, the only thing
   I get is a box with text “Are you sure you want to do this?”.
 * I have tried putting `define('WP_DEBUG', 'true');` but to no avail. I have added`
   php_flag display_errors on` in my .htaccess but I don’t get any error message.
 * Any help is greatly appreciated as I am beginning to doubt my sanity here.
 * Configuration:
    Apache 2.2, PHP 5.2.6 Wordpress 2.9.2 MySQL 5.something
