CAPTCHA is not shown when joining the affiliate program

I’ve just installed Affiliates 2.4.0 in a Word press 3.5.1 site.

Unfortunately the CAPTCHA is not shown when someone tries to register to become an affiliate.

Does anyone know how to fix this?
