Hi there,
After you enable Categories you should then see the Categories under the “Donations” menu item where you can choose to “Add New”. Or, you can go to a single Give form and look to the right-hand sidebar and you’ll see the Categories metabox there where you can add whatever Category you like and add your Give form to that category.

Thanks for the response but I followed all of the documentation. I have the meta box on the form enabled, still I don’t see the categories on the form itself. I really need to be able to have my donors select categories, and would like to get this up and running soon.

I see… I misunderstood your request. You want your donors to be able to choose a category to donate towards. Categories is something a bit different. What you’re looking for (if I understand correctly now) is the multi-level donation option.
Simply go to your form and the first choice you choose should be “Multi-level Donation”. Then you’ll see the ability to Add Rows and designate a label for each Level. Here’s a screenshot:

That will give you the ability to allow your donors to choose the level they want to donate to.

Not exactly. I would like multi-level donations for multiple categories.
Ex:
University 1
– $10
– $20
– $30
University 2
– $10
– $20
– $30
Where Universities are the selectable categories.
I see. Doing that in one form is not currently possible. Instead, I would encourage you to give each university it’s own form, and then use the Archive page as your main Donation Landing page, which would show all your universities on one page, and when the donor clicks on one they are taken to that specific form customized to it’s specific levels.
Here’s an example of a Give archive page which does exactly that:

That example has been styled and customized but the functionality is exactly what I’m suggesting.
