Hi! How is it possible to change the title (‘Write a new entry for the Guestbook’) and to change the input fields?

Hi,
The heading can be changed in the settings.

For the input fields, there are filters. It is not really designed to be just anything. But you can take a look at the filters for the form. There are examples:
