Thank you for getting back to me. The issue is still happening.

It use to show my custom forms but now shows nothing but the red info box shortcodes.

Can you look in your theme folder and see if you have a file named
“wpsc-shopping_cart_page.php”
If you have this file in your theme could you try renaming it to
wpsc-shopping_cart_page.php.off
and then go to your WordPress admin SETTINGS > STORE > PRESENTATION and click “Flush Theme Cache”
Then check your checkout page?

@edward if you want my assistance on this issue i am around today, but i’m sure you will figure it out. i would like to understand the root cause of the problem so i can update the wpec site checkup plugin to detect and perhaps correct the condition. seems like it pops up from time to time.

I appreciate your help. No upgrades or anything recently. The owner of the site did try and add a page under the main Table Registration page as a child, but I deleted that and then it was working for a few minutes, but then again not.
I am running the latest WP 4.0.1
WP e-commerce version 3.8.9.4

Has there been any resolution to this issue? I was only trying to figure out why the tax rate wasn’t working on the plugin, and ONLY working on the tax rate page when the entire checkout form disappeared when I went to check if it was working properly. WordPress 4.3.1, Version 3.10, prairiegodmothers.com
Edited to add that upon further testing, the form disappears only if the shipping zip is in the same state as the shop’s address.
