Conditional fields does not work

I would need a “look from the outside” on a form with ContactForm7 + Conditional Fields. At one point there is a problem and the function of the conditional fields does not work. Unfortunately, I don’t see any faults myself.
Should someone have a short time today to take a look at it with me, I would be happy. 😊
