    I have install usp plugin and everthings seems to be working except
   the submit form doesn’t work as in this [picture](http://prntscr.com/ibm8ac).
   any help would be greatly appreciated. Thanks!

   fields? Is that what you are asking about? Or is there something else, let me

 * Hi tooanhvux,
 * USP does not provide visual styles, but rather leaves them up to your theme. 
   So in this case, the best thing you can do is ask your theme developer for help.
   They will best know which rules need modified in order to add borders to form
   fields, buttons, and so forth. Let me know if I can provide any further infos,

 * Thank Jeff, before asking help from the developer should I copy two plugin files
   which are usp.css and submission-form.php to my theme local directory, and change
   Form style” to “Custom Form + CSS” as guided in the article. Thank you!

 * Yes that may help, just follow the steps if you would like to set up a custom
   form.
