Custom checkboxes

I’m wondering if the following is possible. When a user is leaving a comment, is it possible to have two checkboxes which if checked (one or both) a notice will appear with their comment that shows they checked those boxes? An icon or text would appear below their comment or beside their name. Thanks.

Example:
A checkbox labelled ‘I attended the event’ that will put ‘Attended’ (or an icon) in the comment
A checkbox labelled ‘I purchased a product’ that will put ‘Purchased’ (or an icon) in the comment
