 * Hello,
 * I added 2 custom checkboxes. When someone signs, how do I see in the backend 
   which custom checkbox was ticked and which was left unticked?
   Second question,
   can I remove the thank you message so that after a person signs, the form is 
   displayed again right away rather than the thank you message?
