 * Thanks for your work on this plugin. I have a few more questions for you. I’m
   trying to set up a form for a client and have never used Salesforce before. I’ve
   been working on it all week and they are getting impatient to have it done. Argh.
 * I’m using the fork with different field types like check boxes and list boxes,
   etc.
 * How do I use the Value and Options fields in the form setup? The field name and
   label make sense to me, just not the value and options.
 * I see from another forum post that for a drop-down list I need to type all the
   options with a pipe separating each item, and I got that to work. But what is
   the Value for in this case?
 * How do I get a check box to show it was checked by the user when the form is 
   submitted? Do I put “true” in the value field?
 * How do I make a check box checked by default when the form is loaded on the page?
 * Do you offer consulting for help with this plugin?

 * This explains the format for the additional field types. Checklists should operate
   the same way.

 * Value is if you need the submitted value (in this case, the field value of the
   item chosen, not the confusingly titled value field) to be different than the
   field label (otherwise it uses the field label as the field value).
 * The form UI needs an overhaul… 😉
 * Checked by default… that’s not possible in the current plugin, sorry! You could
   use jQuery to auto check a box, but that’s a hacky workaround.
