Does this plugin exist???? (Lightbox with Contact Form)

I’m looking for a lightbox plugin for my photography site. This plugin may be too good to be true, if it exists:

I need a lightbox plugin that will allow for a ‘contact me’ link or button under each image when it pops into view.

More ideally (but not required) once this link is clicked, I’d like a form to pop up allowing the user to send their contact info, along with a thumbnail (or image name) of the photo they’re interested in.

Anyone ever heard of something like this? If you have, you’ll be my hero.
