# Does WordPress have this ability?

 * I am an office assistant at a company that provides home care to seniors. Our
   caregivers (employees) that go out to provide care have certain tasks that need
   to be done each visit. I am wanting to know if WordPress has the ability for 
   me to create a personalized website for the company that would function as a 
   work place form to be filled out after each visit. More specifically, I would
   like to be able to have the website be a web page that our caregivers would go
   to after each visit and select little check boxes next to the tasks that they
   performed that visit from a predetermined list of tasks we have. I would also
   like the website to have a notes section where they could provide further detail
   or add any other information they think is relevant. I would then like the web
   page form to have a submit button that they would click when done, and when they
   click submit the form would either be stored on the website or have it sent to
   us in an email for review. Please let me know if WordPress is a website builder
   that can help me with this, and if not is there any service anyone knows of that
   fits my above description. Thank You!!!

 * That sort of thing can be done with WordPress, but it’s not built-in straight
   out of the box. It would need to be either a mix of [existing plugins](https://wordpress.org/plugins/),
   or some custom development.
