Doesn't Update Links on Replace

This plugin (great idea!) was installed on a site that I inherited recently. It is now only partially working – if I replace a file using the “update title and replace all links” functionality it now replaces the file and uses the new name but DOES NOT update the link. Also the plugin is not showing in the Plugin menu in the Dashboard, although I can see it in the plugin folder and the “Replace Media” button is still there in the Media Library and attachment edit screens.
I have installed WP Better Attachments to be able to generate lists of attachments and see the attachments on the post edit screen. Is there a compatibility issue?
Does it make a difference whether the file is attached originally through “Add Media” or “Add Attachments”? In most cases I can’t use “Add Attachments” because I need to generate a link to the attachment page.
