Hi- I was having the exact same problem and found that when I set in-depth reviews to ‘no’ that the problem went away. I am not sure if that will work for you if you need that aspect of the plugin but just FYI that is a work-around…
The issue is corrected in the currently-available version 2.0.36.

It sounds like the recent update may not have fully installed (or the files got corrupted somehow).
To fix this:
You need to delete the Ultimate Reviews plugin and do a fresh install of the newest version.
(Note: you won’t lose any data as that is stored in the database)
I actually had the same issue – updated to 2.0.36 and still had the duplicated field. Removed the plugin completely and re-installed, this fixed the issue.
