I tested the issue on our lab sites but could not replicate.
Please confirm that you are using v1.24.6 and try again after performing full plugin/theme conflict test.
Please make sure you have the recent backups and perform this operation on your dev/staging site if possible.

I can confirm that my WordPress version, themes and plugins are all up to date. My forminator is indeed on v1.24.6
I disabled all plugins and tested (Forminator being the only plugin) – problem persists.
I then used a different theme (2021) and tested it – the problem persists.
I must add that you have to start testing on a different page than the form is on, i.e. I was on the Home page, and then click the “contact” button on my menu bar which navigates me to the page with the form. From there, when I complete the form, the {embed_url} returns me the home page link, instead of the contact page link.
If I start the testing on the page I have the form on (i.e. enter the contact page URL in my browser instead of navigating to it from the same domain) then it does return the correct URL.
I’m not sure if I’m explaining it properly – I could record a video for you if you have a secure link for me to send it to?

Thanks for providing more info, however, I’m afraid, I couldn’t replicate when I follow the exact steps.
Possible to share the form export, so that we could compare the settings and see what’s being missed?

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.
You can share the export file via Google Drive, Dropbox or any cloud services in the next reply.
