Sorry to hear about the issue you’re facing with Forminator. * I was able to replicate the issue on your site, I noticed that the form is rendering partly (not all fields are rendered) when I checked in the web-inspector console. It would only mean there is a conflict which is causing the issue. Running a conflict test would be ideal if you are looking to rule out the core issue * If you still face issues even after that then would recommend you open a new [thread](https://wordpress.org/support/plugin/forminator/#new-topic-0) with us, so that we can check further if needed.
