 * i add the fields following instructions on plugin documentation. But when i click
   submit these fields throw some validation and form submit is not completed.
 * any idea why appear the wrong validation?

    You’ll need 
   to change the ‘type’ of your input from ‘date’ to ‘text’. When having the type
   set to ‘date’ it won’t allow time values and the validation fails. Greetings,
