It appears you have a conflict issue related to your cache plugin LiteSpeed cache since I can’t see the form on your site and its not loading at all, some Javascript errors are showing the browser console logs.

I believe the issue is related to the litespeed page settings like Javascript minify, to fix this you need to exclude Forminator files from the optimization features.

Another solution is to exclude the form page from optimization features.

Hi @bagaa,

The form is now loading fine now and I was no longer able to notice any javascript errors in browser console.

However, as the from PayPal is now in live mode, I was unable to test this further at my end.

Can you please share an export of the form using Google Drive or Pastebin.com so that we can check the issue using a test website at our end?
