The link you provided goes to your form edit screen which I cannot access. Most likely this is due to a plugin or theme conflict. I’d suggest using the Health Check plugin to troubleshoot that. It will allow you to deactivate all plugins and put your site on a default theme ONLY for yourself — your live visitors will not see that. We have a write-up on that here:

Then you can enable plugins one at a time and test again until you see the problem again. Then let me know which plugin caused the conflict and I can take a closer look at replicating the issue for you.

but my problem is on the dashboard trying to create a new form on give, on health check mode i can’t test if it’s working it’s hiding all the plugins on the dashboard,

You can, actually. See the tutorial video, it shows how you can enable Give during Health Check mode for testing things like that.
i have watched the tutorial video but i don’t have the plugin option on the said menu to try to add a new form when i’m on troubleshooting mood

in the end, i’ve deleted all my plugins and start again, but it seems that give have problem with Yoast SEO plugin, and this is a plugin we can’t remove, any solution?
Almost ALL of our users use Yoast SEO and we don’t have reports of this. Most likely it’s a combination of Give + Yoast + Some other plugin. It’s important when testing with the Health Check plugin that you test with ONLY Yoast and Give, or ONLY GIve and the other plugin.
