I am having the same issue with the select list, the background is clear and shows the form behind the list.
The radio selection does not show, however the function does work.
I have tested this on firefox and brave, same on both.
I have tested this on IE and almost all of the form does not work at all.
I have manually selected the form background, nothing fixes this issue.
I have tested other form plugins and they work perfectly but would rather continue with forminator.
I hope you can help with this.

I’m very sorry to hear that you’ve experienced this issue while using our plugin!
I wanted to check the page, however I found that the form is not showing up at the moment on that page due to a caching issue most likely so I couldn’t check it further yet.
From the screenshot it looks like the issue is due to conflicting CSS styles between the theme/builder and Forminator, so should be fairly easy to solve once we see the form.
To fix this part, please set the form to Load using Ajax (Behaviour tab of the form’s edit screen) and clear caches on the site, then let us know here it’s done.
@yahsusdenom to avoid mixing the issues even if they seem similar (still solutions may be different due to different theme for example) and notifying the other person with each reply, please create a separate ticket and our team will jump in to assist you.

Thank you, I have done as you have suggested but am still having the same issues.
The form created is on page https://yahsus.com.au/consitution-agreement/.
I have changed the theme many times(before posting above) to see if it was a theme conflicting, then I ended up installing the original theme, Fluida/reservoir, which still works perfectly on another site I have, but the form does not work at yahsus.com.au
The theme I am working with now is Neve/Nevark

I can see your Form style is loading from HTTP but I can confirm your WordPress setting is set to HTTPS.
This is causing the issue as Browser is blocking the Style request.
Can you please try to use this plugin https://ww.wp.xz.cn/plugins/really-simple-ssl/ and see if it will make any difference?
It has some options to force the HTTPS.
