    follow given instruction to change forma field label name go to in your plugin
   directory >> appointment-calendar-shortcode.php
 * open appointment-calendar-shortcode.php and go to line number 993 and here you
   can change your form field label text like name,email,phone, special instructions
 * let me know did it work
