    what is making the structure of this form from getting collapsed.

    What do you mean with the layout collapsed? I visited your web page, and the form design has not varied. But please, keep in mind an iframe tag is like a browser embedded in the page’s content. The design of the iframe content is determined by the page loaded into the iframe.

    The same page of the form is getting overlaped thrice.

    The iframe loading is affecting the form generation. I released an update right now that should prevent this happens. Please, install it and let me know if the issue persists.
