 * What do you mean with the layout collapsed? I visited your web page, and the 
   form design has not varied. But please, keep in mind an iframe tag is like a 
   browser embedded in the page’s content. The design of the iframe content is determined
   by the page loaded into the iframe.

 * The iframe loading is affecting the form generation. I released an update right
   now that should prevent this happens. Please, install it and let me know if the
   issue persists.
