 * We’re getting some complaints like this one below from
    someone who tried to 
   complete the form. Please advise. thank you.
 * The message on my screen says:
    Warning: move_uploaded_file(/home2/manhaua7/public_html/
   http://.manhattanarts.c om/wp-content/uploads/form-maker/Zhelyazkova_R_4 Elements-
   5.jpg): failed to open stream: No such file or directory in /home2/manhaua7/public_html/

 * Could you please make sure the permissions of **/wp-content/uploads/form-maker/**
   folder is set to 777?
