Apologies for the slow turnaround here.
The problem there is likely a conflict between the theme’s styling and GiveWP’s. Most likely (if it’s only happening on the modal or button mode display of the form) the theme has some custom styles for modal popups, and the CSS for the theme popups is either not specific enough or too specific, and inadvertently targeting elements on GiveWP modals as well.
We’re happy to take a look at it, and the fastest way to get this resolved is for us to get a link to a live site where the problem is happening. Since CSS is accessible from the browser, we are then able to see exactly where the conflict is, and most likely provide you with a snippet that fixes it.
