I see you are using plugins on your WordPress to manage the website’s cache, and optimize it. Please, go to the settings of these plugins, and disable the option that optimize the generated HTML code and the inline javascript. And please, remember to clear the website’s cache after modify these plugins’ settings.
If the forms are loading fine when you are logged, it is because these plugins always show fresh versions of pages, if you visit them as a logged user.

Thanks for your suggestions! I went into my plugins and verified that “aggregate inline JS” and “optimize HTML code” were already unchecked in Autooptimize. I excluded CFF from Async Javascript. I then tried disabling both of those plugins. I don’t see that I have any other cache management or optimizing plugins installed. I cleared my cache manually. Still, my forms do not show up. Any other suggestions? Any other plugins I should check?

Please, try the following steps:
– Go to the settings page of the plugin.
– Select the “Direct” option for the “Script load method” attribute.
– and press the “Update” button.

It was already in direct, but I tried switching it to classic and then back again with no effect. Other suggestions?
I disabled all of my plugins except CFF and started from scratch, and my calculator did appear on my public page. So there is a compatibility issue. The forms seem to not appear when either Async JavaScript or SG Optimizer are enabled. Neither have a toggle for optimizing inline Java or HTML. I contacted Siteground as well to see if they had any suggestions. Are you aware of any other settings I should try to change?
