 * The forms generated are awefully weird,it has too many spaces, and it comes in
   a ‘box’ that you need to scroll through horizontaly.
    That does not sell well.

 * Please check how you inserted the shortcode. It looks like you wrapped it in 
   a code block in your back-end editor.
 * Try to edit that page, switch to the `Code` editor and make sure the shortcode
   is inserted as is, without anything around it.
