# Forms Table Missing

 * Hi,
 * I have been having an issue for a couple of days with the plugin. However, I 
   am now getting the following error message
 * It looks like the forms table is missing. Please de-activate and re-activate 
   the plugin to attempt to create the table. If the error persists, please get 
   in touch with the YIKES Inc. support team.
 * I have deactivated and re-activated, I have also uninstalled and reinstalled 
   but nothing seems to resolve the issue.
