The built in get_avatar() feature seems to be generating an incorrect md5 hash; so isn’t retrieving the correct Gravatars. For example, the md5 for my email address is; 08cab17d20de8d790a0fdaad3146da98; but WP generates: cfc6467002ed3982abbe8d6e5481c03c. I have the correct email address( for the Gravitar) set as my email address in General Settings, and have even checked the comments themselves for the correct email address. If this works, re-activate the plugins one by one until you find the problematic plugin(s). * – switching to the Twenty Eleven theme to rule out any theme-specific problems. * – [resetting the plugins folder](http://codex.wordpress.org/FAQ_Troubleshooting#How_to_deactivate_all_plugins_when_not_able_to_access_the_administrative_menus.3F) by FTP or PhpMyAdmin. I had previously switched to the default theme Twenty Eleven but tried again after resetting the plugin folder.
