 * Is there a way to insert a “custom HTML code” after (or before) some chosen offset
   grid indexes? My intention is to insert some “adsense boxes” into the popular
   posts grid.

 * Sorry for the late reply.
 * No, there’s currently no way to do that with the plugin. It’s not impossible 
   though, I will share some ideas with you sometime this upcoming weekend so you
   can insert ads before/after specific indexes.
