Hw do I how make a text box entry?

This is what I want, for any person online who comes on the page to enter;

NAME:

COMMENT:

[ENTER]

And when they click enter it gets posted onto the wall. Very simple but impossible to find!!! Please please please please help me somebody I have spent soooooooooooo long trying to work this out, hours and hours over days and days, but even though i have learnt alot I still cannot do it!!!!! I want to cry….
