how to use same map multiple time in website

I use google map for my wordpress site using “google maps bank” plugin at http://3weekdiet.akssol.com and i set it on bottom of page. Now i need to set same map on http://3weekdiet.akssol.com/index.php/location/ but it shows error “default not defined”.
