 * None of the “Enable support” options, “auto insert” or post level “insert table
   of contents” are selected but the plugin adds html code to the pages, in the 
   form of
 * <span class=”ez-toc-section”……
 * Is it possible to remove this or make it optional.

 * Ok, thanks … I looked into this. Now I remember why this is done. Even though
   the options are disabled, the anchors are still added. This is done in order 
   to support the TOC widget because the widget can still be placed in the sidebar
   regardless if the option to insert the toc is enabled or not so the anchors have
   to exist. Hope that make sense!
