When I enter the details and add the captcha it gives me an error saying that I have entered the wrong captcha value. However I know that 5 + 9 = 14. * Have you tried a WordPress default theme like Twenty Thirteen or so just for testing? I suspect it is a theme problem and not a plugin problem.
