I cannot add form shortcode for appointment section | ww.wp.xz.cn

In the “make appointment” section the space where the form shortcode should go it’s not there, so I cannot add my form shortcode.

Please advice,

Hi there,
Thank you so much for considering our products .

For the Form Shortcode field, You have to install plugins Ultimate Form Builder.
Please follow the link here : https://accesspressthemes.com/wordpress-plugins/ultimate-form-builder-lite/
Then Please Go to ->Admin dashboard->Form-> Create new Form then copy the short code and pest it on Book And Appointment Section .
