# I'm getting two (2) Forms

 * I’m just using a cut-n-paste of your sample “Example of default media upload 
   form” from your FAQ page and I’m just modifying the display values and I get 
   TWO forms on the Post page…
 * I just want one form

 * hmmm… if fact if I just cut-n-paste your exact example; I get two of everything.
 * 2 Title fields, 2 Description fields, 2 Buttons to Choose Files, and 2 Submit
   Buttons,
 * Only the ‘Author’ has one Field

 * suppress_default_fields=”true”
 * seems like that would be a given if using these Shortcode parameters

 * There’s actually a global setting for suppressing default form fields in plugin’s
   settings.
 * And FAQ says:
    Example of default media upload form Here’s example of default
   form (you don’t need to enter all that if you want to use default form, just 
   use [fu-upload-form]):
 * I admit that docs don’t make it very clear, I’m working on better docs
