# Image Based Form

 * I wanted to add a form with radio buttons and check boxes for certain question.
   But I want to use different images for each option offered in radio button and
   checkboxes.
 * Is this possible?

 * You could use the Ultimate Member hook:
 * “um_{$key}_form_edit_field” where $key = your radio/checkbox field metakey
 * and customize the fields HTML on edit or view mode at the UM Form.
