As far as I see – you are using MailChimp as subscribers destination, and you didn’t selected any subscribe list for your subscribers. You must enter your MailChimp and select subscribe list in admin area -> PopUp Edit screen -> Design -> Subscribe to be able use it.
When I am looking at that screen, there is no box to select the list
